NHS Continuing Healthcare
When someone's care needs are mainly health needs, the NHS may pay for their care in full, at home or in a care home. It is not means-tested.
What it is
NHS Continuing Healthcare (CHC) is a package of care arranged and funded by the NHS for adults with ongoing, complex health needs. If someone qualifies, the NHS pays for their care, and their income and savings make no difference (NHS).
How the assessment works
The checklist
A nurse, doctor, other healthcare professional or social worker completes an initial checklist to see whether a full assessment is needed.
The full assessment
A multidisciplinary team of at least two professionals from different healthcare professions assesses needs, using the national Decision Support Tool.
The decision
You should be given the decision in writing, with clear reasons, and a copy of the documents.
Fast track
If health is deteriorating quickly and someone is nearing the end of life, they should be considered for the fast-track route.
Preparing for the assessment
- Keep a diary of care needs for a week or two, including nights
- Gather letters and reports from the GP, hospital and therapists
- Ask to be told about the assessment, and to take part in it
- Describe the worst days, not only the good ones
- Ask someone who knows your relative well to be there
If the answer is no
Ask for the reasons in writing. You can ask the local NHS body that made the decision to review it. If care was paid for during a period that was never assessed, you can ask for that period to be considered; these requests are usually only considered for care after April 2012.
Choosing your carer if CHC is agreed
Ask the NHS team whether a personal health budget could let you choose how care is arranged, including choosing your own carer.
